AMY T PILATES, INC.  WAIVER OF LIABILITY AND INFORMED CONSENT RELEASE

As a participant in Pilates Sessions taught by Pilates Instructor Amy Taravella at Amy T Pilates, Inc studio, I understand I will be taught a Pilates methodology centered in safety via learned practices of precision and control. I will do my best to closely follow the safe guidance of Amy Taravella in my lessons. I understand I will be exercising on Pilates equipment and with props. I am aware that these fitness activities involve an inherent risk of injury. I hereby release Amy Taravella and Amy T Pilates, Inc and the studio facilities of any causes of action, claims, demands, damages, costs, expenses and compensation for damage to myself that may be caused by any act, or failure to act, or that may otherwise arise in any way with any services received. I do hereby waive, release, and forever discharge Amy Taravella, Amy T Pilates, Inc, and the studio facility from all responsibilities or liability from injuries or damages resulting from my participation in the above-mentioned activities. I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved.
